We value collaborative care and strive to provide timely, comprehensive evaluations and evidence‑based treatment. This page outlines how to refer a patient and what to expect from our assessment and therapy processes.

How to Refer a Patient

Referrals may be submitted by physicians, nurse practitioners, physician assistants, mental health providers, and other licensed professionals. We accept referrals for neuropsychological evaluation, psychological evaluation, and psychotherapy services.

What to Include in a Referral

• Reason for referral
• Relevant medical, neurological, or psychiatric history
• Medication list
• Recent imaging and laboratory results
• Prior evaluations or treatment summaries
• Contact information for care coordination

What Providers Can Expect

We prioritize clear communication and timely reporting. Following an evaluation, referring providers receive a comprehensive report that includes diagnostic impressions, treatment recommendations, and guidance for ongoing care. For psychotherapy referrals, we coordinate care as appropriate while maintaining patient confidentiality.

Urgent or Safety‑Related Concerns

If a patient is experiencing acute safety concerns, such as thoughts of suicide, thoughts of harming others, hallucinations, or delusions, it is important that they receive immediate evaluation. Providers may direct patients to local crisis services, emergency medical services, or the nearest emergency department.
